LIABILITY WAIVER & RELEASE

In consideration of being allowed to participate in Single Parent Provision activities, events, and programs, I acknowledge, fully understand, and willingly agree that some activities carry inherent risks. I knowingly and voluntarily assume all risk, both known and unknown, even risk from the negligent acts or omissions of others, and assume full responsibility for my participation. I waive any and all claims against and fully release from liability Single Parent Provision for any damages, claims, injuries, losses, or lawsuits resulting from my participation in Single Parent Provision programs. I further agree to indemnify and hold harmless Single Parent Provision, including its leadership and members, from any harm or damages that may occur directly or indirectly by my participation.

I give Single Parent Provision the irrevocable right to use, reproduce, or publish my name and likeness, in all forms (including photographs and video) and all types of media, for any lawful purpose. I also waive any right to approve or inspect the finished product, including written copy. I additionally waive any right to compensation arising or related to the use of my name or likeness. I have read and fully understand this release of liability waiver and release agreement.

If participation in activities requires a background check and I have completed one with Single Parent Provision before, I give Single Parent Provision permission to use previous information given to re-run my background check.
